Birth Certificates

• The person named on the birth record if 18 years or older
• The parent(s) of the person named on the birth record
• A person with a New York State court order
• A municipal, state, or federal agency when needed for official purposes

Identification requirements (any one of the following)

• Valid driver’s license
• Valid non-driver’s ID
• Passport
• Naturalization papers
• Military ID
• Employer’s photo ID , or
• Police report of lost or stolen ID
• Two utility bills, showing applicants name and address

Processing fee

$10 per copy

Processing time

An application is processed the day it is received by mail or in person. Sorry no fax, telephone or internet requests are accepted, an original signature is required.

If requesting by mail please send in application form, copy of id and remittance payable to: Village of Mount Kisco. Do not send cash through the mail
